The Amyloid Clinical Care Coordinator acts as a liaison and point of contact to patients by helping them access healthcare outcomes from doctors, hospitals, and others who provide their required services. Consistently seeks to improve healthcare experience and positive patient outcomes and eliminates barriers to patient’s access to healthcare services. Facilitates continuity of care/care coordination by guiding patients and families through the health system from diagnosis, testing, treatment, and follow-up care. Organizes and submits data to ensure compliance with regulatory guidelines and for program monitoring and reporting to leadership

RESPONSIBILITIES:
- Acts as a liaison and point of contact to patients by helping them access healthcare outcomes from doctors, hospitals and others who provide their required services.
- Provides a personal, single point of contact for patients and their family. Coordinates referrals to support and ancillary services as needed, and ensures patient has appropriate access to health care services and providers.
- Coordinates orders for appropriate tests, treatments, medications, consults and facilitates scheduling as needed.
- Interprets lab, imaging, and other pertinent data per established protocol and within the clinician’s scope.
- Guides patient and family through the health system from diagnosis, testing, treatment, and follow-up care.
- Collaborates with multidisciplinary team to address patient questions and responses about the care process and treatment options.
- Seeks to improve healthcare experience and positive patient outcome. Ensures patient's satisfaction with the integrated health care delivery model and access to appropriate care or facility.
- Participates in obtaining quality outcomes through data collection and reporting and assisting with patient care compliance.
- Develops and maintains reliable systems to document, track and monitor patient data and progress through the process.
- Completes documentation of the plan, orders and outcome related to the pathway according to policy; communicates verbally or in writing to other team members.
- Demonstrates respect and understanding of other clinical disciplines and uses an integrated approach to achieve desired patient outcomes.
- Facilitates communication among departments to ensure continuity of care, establishes and cultivates relationships with physicians and health care team members. Keeps care team members informed of patient progress and changes.
- Provides and promotes patient and family education and clinical staff education and involvement.
- Facilitates and coordinates scheduling of clinic consults and follow up appointments.
- Any symptom related calls and/or recommendations outside of the clinician’s scope will be delegated to an available RN or discussed with and handled under direct supervision of a provider.
- Enters data in the EHR, scans testing as needed for prompt continuity of care
- Works as a resuscitative team member in emergency situations.
- Upon successful completion of the Prescription Refill Authorization Competency Exam, authorizes prescription refills under direct supervision of the provider as needed.
- The amyloid clinical care coordinator orientation is for one month. The orientation includes all the listed duties above. Orientation is a progressive process.
- The clinician is expected to attend training sessions of new Virginia Heart processes as they are implemented.
- Continuing education is not mandatory but is encouraged and expected.
- Opportunities for continuing education are ongoing and made available to clinical staff.
- Reports general medical supply needs to the office manager. Maintains and cleans clinical equipment per manufacturer’s recommendations. Notifies office manager of equipment failures in a prompt manner.
- Expected to read, understand, and follow the Virginia Heart Employee Handbook, the Clinical Policy, and Procedure Manual, including updated policies when indicated.
- Demonstrates professionalism in all interactions with patients, providers, and fellow employees to create a positive experience. Adheres to the Virginia Heart dress code policy. Exudes confidence and positive body language.

- Maintains workstation and all clinical areas in a clean, tidy fashion.
- Provides exceptional customer service in accordance with corporate standards and expectations. Exhibits empathy and compassion and maintains a professional and positive attitude with customers and fellow employees.
- Respects and maintains patient privacy and dignity. Adheres to HIPAA law and is always aware of one’s own environment and communication with patients.
REQUIREMENTS:
- LPN with prior coordinator experience preferred.
- 2- years of clinical experience in the management of cardiac patients preferred.
- Current Basic Life Support (BLS) required; Advanced Cardiovascular Life Support (ACLS) preferred.
- Computer Literate, proficient in Excel preferred.
- Excellent written and verbal communication skills and organizational skills.
- Strong ability to work independently and to multi-task in a fast-paced environment.
- Prior experience with the use of an Electronic Health Record (EHR), EPIC preferred.
