Advanced Practice Provider/Nurse Practitioner/Physician Assistant -Structural Heart

Location: Loudoun, VA 

Status:  Full-Time 

 

At Virginia Heart, we are looking for an experienced Structural Heart Program APP in collaboration with and under the supervision of the Structural Heart Disease Interventional Cardiologist will evaluate and assist in the medical management of patients with Structural Heart disease. The APP will evaluate and assist in the management of patients, pre and post TAVR, mitral clip, ASD/PFO, and other procedures performed by the Structural Heart Interventional Cardiologist. The APP will be part of a multidisciplinary care team practicing in the inpatient and outpatient setting. As part of the Structural Heart Program. RESPONSIBILITIES:

  • Obtain medical history and perform physical examinations.
  • Perform clinical assessments and develop treatment plans.
  • Initiate, monitor, and adjust medications in accordance with the APP Practice Agreement.
  • Manage patients with complex cardiovascular diseases.
  • Order and evaluate laboratory, diagnostic and radiographic tests.
  • Complete medical records and billing at the end of each day.
  • Coordinate care with rounding physician or office-based physician.
  • Provide education to patients and their family members.
  • Provide pre and post procedural education for patients scheduled for cardiac testing.
  • Make appropriate referrals to community and other interdisciplinary providers.
  • Refer to appropriate Virginia Heart Programs: for example: Heart Failure Program, Research Program, Sleep Program.
  • Act as liaison for the physician in communicating with patients and family members.
  • Regularly attend APP & Provider Meetings.
  • Perform daily hospital rounds, coordinate daily work with the rounding MDs at the beginning of day, prioritizing patient care based on severity of illness, new patient consults/admissions and providing timely discharges.
  • Maintain communication with the rounding physicians. Communicate unresolved issues to the MD at the end of the day.
  • Maintain hospital list. Coordinate scheduling of inpatient stress test, Cardiac Cath and other inpatient and outpatient testing.
  • Provide discharge education and outpatient follow up orders and plan.
  • Maintains own schedule of structural heart disease patients. Assist in the management of patients’ pre and post procedure for TAVR, mitral clip, ASD/PFO repair.
  • Orders appropriate tests and imaging studies for patients with Structural Heart Disease.
  • Provides patient andfamily education regarding structural heart disease and SHD procedures. Engage in weekly multidisciplinary team meetings.

REQUIREMENTS:

  • Graduate of an accredited Nurse Practitioner Program or Physician Associate Program (Master’s level of preparation preferred) 
  • NP Credentialed by the ANCC (American Nurses Credentialing Center). PA credentialed by NCCPA (National Commission on Certification of Physician Associates) 
  • BLS/CPR and ACLS certified 
  • Licensed in State of Virginia 
  • Experience in Cardiology recommended– preferably as an APP 
  • Have appropriate hospital privileges 
  • Able to work collaboratively 
  • Attend CME annually
