The Interventional and Structural Heart Nurse Navigator serves as an advisor and educator, providing emotional support, clinical education, resources, and expert guidance to patients and their families throughout the interventional or structural heart evaluation process.  This role offers real-time, in-clinic support to the structural heart team and their patients, including education and administrative coordination. The nurse navigator helps ensure a seamless patient experience between Virginia Heart and partnering hospitals, supporting continuity of care and promoting excellent patient experience. The nurse may be required to travel to other Virginia Heart offices as needed..

RESPONSIBILITIES:

  • Interventional Responsibilities
  • Serves as a primary point of contact, in collaboration with the interventional navigator team, for patients undergoing peripheral and coronary interventional procedures.
  • Contacts patients within appropriate timeframes based on their scheduled procedure date to provide education, instructions, and support.
  • Maintains a reliable, organized system for documenting, tracking, and monitoring patients throughout the interventional evaluation and procedural process.
  • Addresses pre procedure medication questions and collaborates with operating physicians to ensure patient specific needs and physician preferences are met.
  • Triages patient symptoms to assess urgency and ensure alignment between clinical requirements and procedural planning.
  • Provides patients with clear guidance regarding peri  and post procedural expectations and addresses consent related questions.
  • Acts as the primary point of contact when procedural urgency escalates, including changes in symptoms or new clinical concerns.
  • Conducts post procedure follow up (same day or next day after discharge) to assess for complications, answer questions, and identify issues requiring clinician involvement, in collaboration with the IV APP or IV Doctor of the Day.
  • Identifies patients with potential emergent concerns requiring immediate evaluation and coordinates same day visits or emergency department referral when appropriate.
  • Manages last minute scheduling adjustments by collaborating with operating physicians and schedulers to optimize daily workflow, including identifying available providers, securing catheterization lab support, or rescheduling lower acuity cases as needed.
  • Functions as a liaison with referring providers to ensure timely communication and continuity of care.
  • Accurately and promptly documents patient interactions and clinical updates in the electronic health record (EHR).
  • Structural Heart Responsibilities
  • Supports structural heart providers during patient visits, including chart preparation, pre screening, reviewing diagnostic results, completing STS scores, and following standard rooming procedures.
  • Reviews the structural heart consult work queue and provider schedules to ensure accurate scheduling workflows.
  • Responds promptly to in basket messages, secure chats, emails, faxes, and other communication channels.
  • Maintains a current, organized tracking system for patients engaged in the structural heart evaluation process and facilitates required testing, follow up, and procedure scheduling.
  • Organizes and submits procedural data in accordance with regulatory and reporting requirements.
  • Provides patient and family education, ensuring understanding of the evaluation process, treatment pathway, and procedural expectations.
  • Serves as a communication liaison among treating providers, referring physicians, the valve clinic, patients, families, and referring facilities to support high quality, coordinated care.
  • Collaborates with hospital scheduling teams to ensure procedures are arranged within the expected timeframe and with the appropriate provider.
  • Conducts telephone triage, pre procedure screening, and patient follow up for both pre  and post procedure inquiries.
  • Maintains and updates patient education materials to ensure accuracy and relevance.
  • Orientation, Competencies, and Ongoing Development
  • Completes a six week, progressive orientation covering all aspects of the interventional and structural heart navigator role.
  • Participates in training as new Virginia Heart processes and workflows are introduced.
  • Engages in optional continuing education activities to support professional growth.
  • Shadows interventional and structural heart teams as needed to remain current on evolving procedures and clinical practices.
  • Upon successful completion of required competency exams, independently processes prescription refill requests under provider supervision and manages anticoagulation therapy as needed.
  • Serves as a resuscitation team member in emergency situations, including administration of IV medications as appropriate for licensure and competency

REQUIREMENTS:

  • Successful completion of a Registered Nurse program
  • Current Virginia State Registered Nurse License in good standing
  • Preferred experience or knowledge of general cardiology, valvular disease, surgical and non-surgical treatment options and or catheterization lab experience preferred
  • At least one year of clinical experience, cardiology experience preferred
  • Current Basic Life Support (BLS) required; Advanced Cardiovascular Life Support (ACLS) preferred
  • Computer Literate
  • Prior experience with the use of an Electronic Medical Record (EMR), EPIC preferred
  • Strong knowledge of clinical nursing care procedures and protocols.
  • Basic knowledge of managed care.
  • Strong verbal and written communication skills
  • Strong interpersonal skills
  • Proficient computer skills
